Sports

School sports are a great way for kids to get involved and meet friends! If you child would like to participate in a sport, he/she must have the following three forms on file in our health room. These forms are also available in the office.

- Annual Medical History form - pdf 68kb (yellow)
- Permission to Transport form - in registration packet (white)
- School Sports Pre-Participation Exam form - pdf 44kb (sports physical - green)

A sports physical is required if your child is planning on going out for a sports team and it needs to be on file in the Briggs health room. Bring the School Sports Pre-Participation Exam form (above) to the doctor for the physical. Physicals are good for two years. You may get a low-cost sports physical through the Springfield Schools Health Center, located next to Springfield High School. Please call for an appointment, 744-4130.
