Information
Clinic staff provide
services to Springfield students and family members.
The clinic is located in the former music
building at Springfield High School (entrance is off 10th Street
and Healthy Way, across from Silke Field).
Springfield students needing
routine check ups, well child care, immunizations, ill children,
children without a doctor and those needing comprehensive health
care including sports physicals may access free and low-cost
services at the Springfield School Based Health Center. All
required vaccines will be available at low or no cost at the
Springfield School Based Health Center. Please

for attendance at public schools.
Sports Physical
Exams:
All students participating
in district sponsored sports teams (middle and high schools)
must have a sports physical on file in the school office.
Sports physicals are required every two years with a yearly
update signed by the students parent or guardian. The Oregon
School Athletic Association (OSAA) requires that sports physicals
are only accepted from a licensed M.D. or D.O. (Doctor of
Osteopathy), or a nurse practitioner and that the physical
be completed before the student begins practice.
Sports physicals
are available through the clinic. The examination cost is
$50.00. Contact your school athletic director for information
on financial assistance that is available to help cover
this cost.

Why does Springfield need a school based
health center?
Approximately 35% of the students in our
district don't have access to medical services, most often because
they don't have health insurance or a family doctor.
What Services are offered?
Services available through the school based
health center include: sports physicals, camp physicals, student
work physicals and medication management (i.e., prescriptions,
asthma treatments, blood sugar maintenance, etc.). The staff
can also update immunizations and treat common ailments such
as ear infections, fever, sore throats and rashes. Well-baby
guidance and parenting support for teen parents is also available.
Parents must accompany elementary age students.
Students 15 and over do not need their parents to accompany them.
Oregon Health Plan Sign Up Assistance - Help is available for
families without insurance.

Is there a fee for services?
Students: Sports Physicals are $50. All other services to the
student are free.
If you have medical insurance, we will bill
your insurance company and the co-pay will be waived.
Adults: There is a sliding scale fee for services based on family
size and verified income with a minimum office visit charge of
$20. If you have medical insurance, we will bill your insurance
company and you must pay your co-pay. We also bill Medicaid and
OHP.
Donations to the clinic are accepted.
